Results: The mean age was 59 years, seven (35%) and 14 women (65%) men. Typical cutaneous scar or previous history of cutaneous leishmaniasis were observed in eight patients (38%) and 13 (62%) denied history of leishmaniasis and had no scar. The onset of nasal or oral symptoms until the definition of diagnosis ranged from five to 240 months, an average of 65,48 months. In Montenegro skin test, the average size of papule was 14,5 mm and size did not correlate with the duration of the disease (p = 0.87). Nasal involvement predominated and extension of the lesion in mucosa did not correlate with the duration of the disease. Parasites were found in two (9.52%) specimens of lesions submitted to biopsy. Conclusion: in the evaluated geographical area, diagnosis of the ML was late. Skin scars and cutaneous leishmaniasis prior history were not good clinical markers for diagnosing ML. Otolaryngologic evaluation, Montenegro skin test and histopathologic examination of injuries in the mucosa for exclusion of other diseases remain important markers for the diagnosis of ML.A leishmaniose cutaneomucosa (LM) pode deixar seqüelas graves e estigma. O diagnóstico precoce evita as complicações. Objetivo: Avaliar os marcadores clínicos utilizados no diagnóstico da leishmaniose mucosa. Metodologia: Foram avaliados 21 pacientes oriundos da região metropolitana de Belo Horizonte com diagnóstico confirmado de LM por meio de entrevista clínica, endoscopia nasal, biópsia e intradermorreação de Montenegro. Resultados: A idade média foi de 59 anos, sete (35%) mulheres e 14 (65%) homens. A cicatriz cutânea típica ou história prévia de leishmaniose cutânea foi observada em oito (38%) pacientes e 13 (62%) negaram história de leishmaniose e não tinham cicatriz. O início dos sintomas nasais ou orais até a definição do diagnóstico variou de cinco a 240 meses, média de 65,48 meses. Na intradermorreação de Montenegro, o tamanho médio da pápula foi de 14,5mm e o tamanho não se correlacionou com a duração da doença (p=0,87). O comprometimento nasal predominou e a extensão da lesão mucosa não se correlacionou com a duração da doença. Parasitos foram encontrados em dois (9,52%) espécimes de lesões submetidas à biópsia. Conclusão: Na área geográfica avaliada, o diagnóstico da LM foi tardio. A cicatriz cutânea e história de leishmaniose cutânea prévia não foram bons marcadores clínicos para o diagnóstico de LM.
