Summary:This article builds reflections on the Plan for the Reorganization of Attention to arterial Hypertension and diabetes mellitus (PRAHADM), with emphasis on the trajectory of Health Policy, aligned with the political contexts that permeate the construction of Health in Brazil and the evolution of the causes of deaths, throughout the twentieth century, as well as the close relationship between the State and capital that causes repercussions in Brazilian public health. It is emphasized to understand within this context, the importance of treating Hypertension and diabetes mellitus as the main causes of cardiovascular diseases, responsible for the high rate of deaths in Brazil, in the context of precarious public health. This is a bibliographic study, based on a survey of the literature relevant to the theme, as well as research by VIGITEL, a surveillance system for risk and protection factors for chronic diseases by telephone survey, from the Ministry of Health, between 2012 and 2019
